TM3 and PPS Alternatives for UK Physiotherapy (2026)

TM3 (Blue Zinc) and PPS (Rushcliff) are two of the most established UK-focused systems for private physiotherapy practices. Both are strong on UK workflows and insurance billing. Like every off-the-shelf platform they also have limits. You cannot change them far, you pay for each clinician, and the exercise prescription is shallow. This article gives an honest view of what they do well. It then covers Cliniko, WriteUpp, Jane and Nookal. It ends with the option of a system built to fit your clinic exactly.

TM3 (Blue Zinc)

TM3 is a UK-developed cloud system with a strong physiotherapy and MSK user base. It includes appointments, clinical notes (with sketchpad and customisable templates), billing, online booking, embedded payments (ClearAccept) and reporting. It is positioned for UK private practice workflows and is trusted by many physiotherapists for its MSK clinical records capabilities.

Insurance billing support is present; clinics should confirm the current depth of Healthcode integration for their specific insurers (Bupa, AXA, WPA etc.). Exercise prescription is typically handled via integration rather than a core module. Pricing is a subscription, usually quoted for each user or each site. UK physios name it beside Cliniko and WriteUpp when they want a UK-focused option.

PPS (Rushcliff)

PPS (Private Practice Software from Rushcliff) is a UK system used by physiotherapy and other private practices. It offers appointment scheduling, treatment notes (SOAP), client and insurance billing, online booking, KPI reports and database auditing. It has a documented integration with Rehab My Patient for exercise prescription and explicit Healthcode Electronic Billing support.

Pricing starts from around £15-£60+/mo depending on package and users. Some professional associations have offered extended trials or discounts. It is one of the more customisable UK options, which is a strength for clinics with specific billing or notes requirements. The interface can feel more traditional than newer cloud platforms.

Cliniko, WriteUpp, Jane, Nookal as alternatives

You may use TM3 or PPS and want more. Stronger Healthcode support built in, exercise prescription without a second subscription, or a more modern interface. Four options come up most. Cliniko has tiered pricing and no built-in Healthcode. WriteUpp has Healthcode built in, and charges for each user. Jane has strong notes and Healthcode support. Nookal has Healthcode built in at 0.3% of each claim.

Each has trade-offs on UK billing depth, exercise prescription, and per-clinician costs. The pattern repeats. Off-the-shelf platforms work, and they force workarounds on the details of UK private physio. Insurer billing. Your exact notes templates. Exercise prescription built in. Reporting on missed appointments, room use and revenue for each clinician.

What per-clinician pricing does to a clinic

Both platforms here charge for each user. That single choice shapes how a clinic behaves.

A new physiotherapist raises your bill before they see a patient. A part-time locum costs a full seat. Some clinics then share a login, which breaks their audit trail and their professional duty at the same time.

The price also moves one way at renewal. Your position weakens each year, because your records sit inside the system.

We build clinic systems on engage.re, and we charge no fee for each seat. Your headcount and your bill are separate numbers, so nobody has a reason to share a login. You own the code, the data and the instance, so a renewal is not the only option you hold.

We explain why a rented price rises each year in your software bill goes up every year. We work through the full cost in what does it cost to own and run your own systems.

The bespoke option

Some clinics want three things at once. Insurance billing for the exact insurers they see. Exercise prescription inside the clinical record, with no separate login or fee. Reporting that matches their contracts, with no subscription for each clinician. A system of their own removes that effort. You own the code outright. The system is built around how your clinic actually runs, with HCPC/CSP record-keeping, audit trails and retention logic in the architecture.

If you want to see what a unified system looks like, ESRE's live physiotherapy demo opens without a login. Sign in as a physio and write up a treatment with an exercise plan, or as the patient and see it on your phone. See the physiotherapy software hub for the demo and more.
