Cliniko Alternatives for UK Physiotherapy Clinics (2026)

Cliniko is a popular cloud practice management system among UK allied health clinics, including physiotherapists. It offers clean scheduling, clinical notes, invoicing, online booking and reporting. However, for UK private physiotherapy practices the gaps around native Healthcode insurance billing and exercise prescription (usually a separate Physitrack or Rehab My Patient subscription) are consistent. This article looks at why clinics consider alternatives and what TM3, PPS, WriteUpp, Jane, Nookal and bespoke actually deliver on the UK-specific pain points.

Why UK clinics look past Cliniko

Cliniko is a popular cloud PMS among UK allied health clinics, including physiotherapy. It offers clean scheduling, clinical notes, invoicing, online booking, payments and reporting. The interface is straightforward and the tiered pricing (full features included at every band) is transparent: roughly $45 USD for a solo practitioner, $95 for 2-5, scaling to $395 for 26-200 (billed in local currency equivalent).

However, for UK private physiotherapy practices the gaps are consistent:

  • No native Healthcode integration. Clinics with Bupa, AXA Health, WPA or other PMI volume must use a third-party bridge (e.g. Effra) or submit directly. This means another login, potential double entry, and reconciliation work.
  • Exercise prescription is a bolt-on. There is no built-in video library and adherence tracking at the level of Physitrack or Rehab My Patient. Most UK users run a separate subscription (Physitrack ~£19.49 + VAT per practitioner per month).
  • Per-practitioner (or tiered) scaling. Costs rise as you add clinicians, associates or locums. For a growing clinic this is a real line item.

Cliniko is perfectly functional for many clinics. The question is whether the combination of billing workarounds and a second exercise tool actually fits how your practice runs, or whether you end up maintaining spreadsheets and logins alongside the "all-in-one".

The alternatives

TM3 (Blue Zinc)

UK-developed cloud system with a strong physiotherapy and MSK focus. Insurance billing support is present; confirm current Healthcode depth for your main insurers. Exercise prescription via integration. Quote-based pricing, often competitive for UK physio clinics.

PPS (Rushcliff)

UK private practice software with appointment scheduling, SOAP treatment notes, client and insurance billing, online booking, KPI reports and auditing. Explicit Healthcode Electronic Billing integration. Strong documented integration with Rehab My Patient for exercise prescription. Pricing from around £15-£60+/mo depending on package. Customisable, which suits clinics with specific billing or notes requirements. Good for UK-focused physio practices.

WriteUpp

UK cloud PMS for healthcare professionals including physiotherapists. Scheduling, clinical notes, invoicing, online booking, payments and direct native Healthcode integration for major UK insurers (Bupa, AXA Health, Aviva, Vitality, WPA etc.). This is one of the stronger native UK billing options. Pricing per-user with practice fees: Flex ~£19.95/mo, Solo ~£27.95/mo, Group from ~£45.95/mo. Add-ons for booking and video. Frequently mentioned when UK physios discuss Cliniko alternatives.

Cloud PMS with strong clinical charting, treatment plans, scheduling, billing and client portal. UK presence with documented Healthcode submission support (beta and export flows). Good depth in notes and workflow. Physitrack integration available for exercise. Pricing plan-based with per-practitioner elements on higher tiers. Strong for clinics that value detailed clinical documentation.

Nookal

The bespoke option

For clinics that process meaningful PMI volume or that want exercise prescription as part of the clinical record (not a second login and another monthly fee), the off-the-shelf combination often creates ongoing friction. A bespoke system can include booking tuned to your pathways, your exact notes templates and body charts, built-in exercise prescription, and insurance billing for the insurers you actually see, all from one set of records, owned outright with no per-clinician fees.

If you want to see what a unified system actually looks like, ESRE's live physiotherapy demo opens without a login. Sign in as a physio and write up a treatment with an exercise plan, or as the patient and see it on your phone. It is a working system. See the physiotherapy software hub for the demo and more.
